Under the requirements of the Affordable Care Act (ACA), Form 1095-C is provided to current and former full-time Kent School District (KSD) employees as documentation of the health coverage KSD offered to you and your eligible family members during the applicable tax year.
Important Update on Distribution
Due to a recent change in federal law under the Paperwork Burden Reduction Act, employers are no longer required to automatically mail Form 1095-C to employees’ homes. Employees must now access the form electronically or request a copy.
Current Employees
If you are currently employed by Kent School District and need to access your 2025 Form 1095-C, please log in to Skyward Qmlativ to view and print your form beginning Monday, March 2, 2026.
Former Employees
If you are no longer employed by Kent School District but worked for the district at any time during 2025 and need a copy of your 2025 Form 1095-C, please email employeebenefitscommunications@kent.k12.wa.us.
