• Washington State law requires all students to be fully immunized against chicken pox, hepatitis B, diphtheria, tetanus, polio, measles, pertussis, mumps and rubella before attending school.

    View the vaccines required for school attendance and provide proof of your student’s immunization status on the Certificate of Immunization Status (CIS) form.

    • Four doses of tetanus-diphtheria with the last shot being after age four
    • Three doses of polio with the last shot being after age four
    • Two doses of the mumps, measles, and rubella (MMR) vaccine
    • Hepatitis B series with the final dose being after 6 months old
    • Two doses of varicella (chicken pox) or verification of the disease from a physician
    • Tdap must be received and proven upon entering sixth grade

    Students with exempt status will be excluded from school in the event of an outbreak (one case constitutes an outbreak) and will remain excluded for at least two weeks after the onset of the last case.
