Daily Wellness Screening Form
Answer Health Questions Every School Day
All hybrid students and staff attending school in person are required to answer questions on a daily wellness screening form before entering the building each day to confirm they are not experiencing COVID-19 symptoms and have not had contact with someone who has COVID-19.
Students who have chosen to remain in remote learning do not need to complete a daily wellness screening form.
Step 1: Complete A Form Before School
Check your student(s) for COVID-19 symptoms and answer questions every morning before they leave for school. You will complete one daily wellness screening form for your student(s) based on the school's chosen primary method:
- Method 1: Instructions to complete the printed health verification form.
- Method 2: Instructions to complete the electronic Skyward wellness screening.
Elementary School Students (Grades PK-6)
Parents/guardians answer questions for their elementary student(s) using their school’s chosen primary screening method.
- Printed Health Verification Form: George T. Daniel, Kent Valley Early Learning Center, Martin Sortun, Meadow Ridge, Panther Lake, and Park Orchard, and Springbrook elementary schools.
- Skyward Wellness Screening: Carriage Crest, Cedar Valley, Covington, Crestwood, East Hill, Emerald Park, Fairwood, Glenridge, Grass Lake, Horizon, Jenkins Creek, Kent, Lake Youngs, Meridian, Millennium, Neely-O’Brien, Pine Tree, Ridgewood, Sawyer Woods, Scenic Hill, Soos Creek, and Sunrise elementary schools.
Middle & High School Students (Grades 7-12)
Parents/guardians or secondary students answer questions on the electronic Skyward wellness screening.
- Parents/guardians answer questions for their student(s) in Skyward Family Access.
- Students answer questions for themselves in Skyward Student Access.
Step 2: Send Your Student to School or Keep Them Home
Your answers on the daily wellness screening form determine if your student can go to school.
- If you answered NO to all questions: Send your student to school.
- If you answered YES to any of the questions: Keep your student home, report the absence to your school, and contact your health care provider.
The symptoms listed on the COVID-19 student wellness form overlap with common chronic conditions, including asthma, diabetes, chronic fatigue syndrome, seasonal allergies, life-threatening allergies, irritable bowel syndrome, congenital heart conditions, and more. These symptoms also overlap with common infectious diseases including influenza, the common cold, norovirus (stomach flu), roseola, and more.
A student who presents with symptoms related to a condition that is not COVID-19 may return to school with a negative COVID-19 test or a letter from their healthcare provider indicating that their symptoms are not related to COVID-19.
Step 3: Student Wellness Will be Confirmed at School
School staff will confirm all students have a completed wellness screening form for the day, review their answers, and check them for a fever and other COVID-19 symptoms if needed upon arrival.
School staff members are trained on signs and symptoms of COVID-19 to look for during the school day and at entry for students who may be feeling unwell. They are also trained in how to protect student privacy during re-screening.
If you answered YES to any of the questions and the student has already arrived at school, the student will wait in a wellness room until they can be picked up to go home. School staff will inform the school nurse of a YES answer for follow-up.
If you did not complete the wellness screening form and the student has already arrived at school, staff will call home to request a verbal health check or prompt a digital health check.
If the student has a temperature of 100.4 or higher, they will be asked to step aside and be re-screened with another thermometer. If a student has a verified temperature of 100.4 or higher, they will be brought to the wellness room until they can be picked up to go home. School staff will inform the school nurse of a YES answer on or of fever for follow-up.
